The UNH occupational health program for personnel who handle or work with live vertebrate animals is a joint responsibility of 是不是健康 & 健康, 研究诚信服务(RIS), and the Office of 环境卫生 and 安全 (OEHS).
每位UNH教职员工, or student handling or working with live vertebrate animals at UNH must participate in the UNH Occupational 健康 Program for 动物保健 Personnel. Prior to handling or working with live vertebrate animals at UNH, applicable individuals need to:
- 完成(提供 所有 requested information) a Medical History and Risk Assessment Questionnaire for Persons Handling or Working with Live Vertebrate Animals. Questionnaires missing information will be returned to the individual. 注意: Applicants need to have had a tetanus shot within the last 10 years and a physical within the last 5 years.
- 将填妥的表格交回卫生保健中心 & 健康通过上传文件 在这里. If t在这里 are problems uploading the document, hand-deliver in an envelope or mail the completed questionnaire to: Attn: Animal Handler Medical Questionnaire, 是不是健康 & 健康 4 Pettee Brook Lane, 达勒姆，新罕布什尔州03824.
- If applicable, respond promptly to communication from 是不是健康 & 健康 staff regarding providing missing information and/or obtaining medical services (e.g., 物理考试, tetanus immunization) necessary to receive occupational health program approval to handle and/or work with live vertebrate animals; and
- Wait for notification from RIS to the applicable project investigator/instructor or animal facility manager of approval prior to handling or working with live vertebrate animals.
Medical treatment for employees with illness and injury is provided by 是不是健康 & 健康 and/or the employee’s primary care provider. The occupational health program for animal handlers includes the following:

- Physical examinations and immunizations (at the discretion of the attending physician)
